The therapeutic-index thesis is only partly validated because the study avoided catastrophic or severe psychiatric toxicity but showed a monotonic numerical irritability imbalance across active doses.

Where the market may differ

The post-read variant is not whether CRB-913 works; it is whether modest but reproducible irritability limits chronic-use differentiation. The company emphasizes class-comparable psychiatric safety and favorable GI tolerability, while Clinaptis gives more weight to the selected psychiatric population, dose-patterned irritability and incomplete discontinuation detail.

Supporting evidence

CANYON-1 enrolled 254 participants and produced a coherent 2.8%, 3.3% and 5.0% placebo-adjusted Week-12 dose response across 20, 40 and 60 mg, with p<0.0001 at each dose.
The 60-mg dose delivered 5.0% weight loss despite being only 40% of the 150-mg Phase 1a dose, substantially reducing the prior dose-translation concern.
All 60-mg completers lost weight; 44.4% lost at least 5%, 6.7% lost more than 7.5%, and the maximum individual loss was 13.4%.
No suicidality or serious or severe psychiatric AEs were reported, arguing strongly against a rimonabant-like liability of similar magnitude.
Vomiting and constipation were infrequent at 60 mg, supporting potentially differentiated GI tolerability despite cross-trial limitations.
CRB-701 provides a second clinical value driver.
Human monotherapy efficacy makes GLP-1 combination development materially more credible.

Tempering evidence

Irritability was 0% on placebo versus 6.2%, 8.2% and 9.7% across active doses, a monotonic numerical pattern that conflicts with a fully clean CNS interpretation.
CANYON-1 excluded meaningful psychiatric history, including PHQ-9 above 4, making tolerability easier to demonstrate than in a broad obesity population.
The 5.0% result is an efficacy-estimand MMRM output; treatment-policy sensitivity, missing-data rates and dose-level discontinuations remain undisclosed.
The reported 3.1%–13.1% active-arm AE-discontinuation range is not mapped by dose in the release.
The no-plateau description remains a management claim until the actual trajectory is reviewed.
GI comparisons are cross-trial and cover only 12 weeks versus 36–68 weeks for the cited oral GLP-1 studies; diarrhea at 60 mg was 22.6%.
